HELP!

Ok, as you can see Im trying to make the thing below so that people can type their name and the album they would like to request. I have no idea how to do this type of stuff. If you know how to do this email me.

You need a place that will handle the form, when you use a 'mailto' link it opens the end-user's default email application rather than immediatly send the form to you.
